Everyone wanting to get vaccinated for coronavirus in Austria will need to fill in the following form. It is available here in English.
Coronavirus vaccinations started in Austria in December.
As at March 9th, the vaccinations have been largely restricted to people in risk groups.
From April however, the Austrian government expects vaccinations to be opened to the general public.

While the Austrian government has provided English, Turkish, Bosnian, Croatian and Serbian versions of the form, the official form is only available in German.
It can be accessed and filled in at the following link.
The other versions of the form are only for translation guidance. If you fill them in and submit them, they will not be accepted.
